To save you time on the day of your first visit, please feel free to print out the following documents so that they may be completed prior to your scheduled visit. New patients of any age will need to print out the “New Patient Documents”. If a new patient is under 18 years of age, please also print out the “Pediatric New Patient Questionnaire”. We look forward to seeing you in the office soon!
Our practice consists of five physicians board certified in Family Medicine or Internal Medicine, as well as two excellent certified Physician Assistants (PA-C) to help provide extended coverage and “same-day” appointments. For your convenience, we also employ a full time certified lab technician so that most of your necessary tests can be performed right in our office – some with STAT results. We also have an in-house X-ray suite and a certified x-ray technician for your radiographic diagnostic evaluations. In addition, we admit and care for our patients at Chester County Hospital so that there is no interruption in the continuum of care.
Family Practice
Family Practice is the medical specialty which covers the preventative care and the diagnosis and treatment of both acute and chronic illnesses. It involves the entire spectrum of ages and allows families to have a one physician relationship. Family Practice includes Pediatrics and Women’s Health as well as minor dermatologic surgery and sports medicine. Our care, however, does not stop at our office doors as we admit and care for our patients while in the hospital. We feel that if you have entrusted your healthcare needs to us when you are healthy, so much the more we want to care for your needs when you are faced with illnesses requiring hospitalization. We will also continue that same excellent care after your hospital stay if further rehabilitation is necessary as we follow our patients at most local rehabilitation and skilled care facilities. The practice also provides care at many of the assisted living facilities in the area.
